Health Coach Educator - NY Program
:
•Responsible for delivering new and established program during coaching sessions.
•By assimilating health assessment data analysis, establishes a personalized plan of care based on participant�s health status/risks, education, and support needs.
•Using holistic approach to wellness, derives strategies to overcome participant barriers in meeting goals and objectives in health programs.
•Must be able to discern from clinical review of member profile and or discussions with a member when a Nurse transfer or consult is appropriate.
•Oversight and managing a Que phone line coming from Emblem Health Gold line by direct assistance and consultation or routing to appropriate healthcare or nurse for ongoing counseling to members delivering concierge level service.
•Utilizes motivational interviewing skills to ensure maximum participant engagement and provides coaching, information and support to empower participants to make ongoing independent healthy lifestyle choices.
•Through the use of clinical tools and information/data review, uses knowledge and experience of lifestyle risk behaviors to conduct comprehensive evaluation of participant�s health status, health literacy, lifestyle risks and readiness to change as they relate to health programs.
•Displays ability to follow established program workflow and accurately document participant data into electronic chart in a timely fashion.
•Performs other duties as required.
Qualifications
• Bachelor�s Degree in health related field (e.g., Health Education, Wellness, Health Promotion) - Master�s Degree or Certified Health programs preferred.
• 3-5 years of coaching experience � Health Coach Certification desired.
• Experience with health coaching across modalities � including face to face, telephonic, digital or a mixture
• Bilingual Spanish, simplified Chinese or Polish preferred.
• Background in motivational interviewing, active listening skills, Transtheoretical Model, mindfulness based, and other evidence-based coaching techniques preferred.
• Experience in wellness related areas such as weight management, nutrition, physical activity or health lifestyle behavior change.
• Superior presentation skills required, including ability to deliver content in an engaging way and adapt to audience needs and/or questions.
• Experience in facilitating patient/client positive behavioral change.
• Health insurance background preferred.
• New York travel required occasionally.
• Flexible hours required, including up to 3 evenings per week.
