��Bachelor�s Degree
��Minimum 5-8 years of relevant work experience
��Demonstrated track record in utilizing established tools for managing project scheduling and tracking
��Operational knowledge of Care Management
��Strong organizational / project management skills
��High level of proficiency with MS Office (word processing, database/spreadsheet, presentation, and project management)
��Working knowledge of, and experience with, SAS
��Provide project management support to the AVP of Care Management.
��Manage department and enterprise initiatives focused on the Care Management function within Medical Delivery.
��Complete and distribute data and reports, including the completion and distribution of the Monthly Care Management Dashboard.
��Manage key tasks including the coordination, documentation, follow-ups, identification of risks/issues, communication for all aspects of Care Management.�
��Monitor progress of projects; provide feedback and recommendations; compile and present results of analyses to appropriate parties; and develop and prepare meeting materials that monitor progress.
��Provide project management leadership for existing and new enterprise programs, such as Cityblock, EmblemHealth/AdvantageCare Physician coordination, City of New York HMO Implementation, process redesign/re-engineering, and improvement initiatives.
��Manage and coordinate the day-to-day activities and deliverables of project team(s).�
��Cultivate strong relationships internally with leadership and necessary stakeholders for effective cross-functional collaboration; organize different individuals, stakeholders, and business leaders (internally and externally) around shared goals.
��Work with department leaders and project team members to develop and maintain an up-to-date integrated project plan, and to ensure that objectives, timelines, and key deliverables are specified.
��Responsible for project execution; coordination across external agencies, internal departments and cross-functional project teams; defining and managing project scope; delegating tasks; developing timelines, objectives, deliverables, and project plans; providing updates (including� dashboards and tracking documents) on progress status and risks to ensure that the project stays on track.
��Ensure that appropriate resources have been allocated to maximize chances of project success.�
��Facilitate project meetings, provide status updates; ensure visibility and availability of project information.�
��Ensure proper development, preparation, and delivery of all project related documentation and communication.
��Identify, anticipate, prevent, and mitigate/resolve issues and obstacles that impede project completion.
��Monitor and control project scope; ensure proper change control procedures are followed on all change requests
��Follow up as needed to ensure deliverables are met, and that obstacles have been resolved.
��Develop/participate in the development of change communications relative to the project outcome.
��Coordinate ongoing team activities - budget, communications and organizational updates, staffing, internal and external meetings.
